Linkage to routinely collected information

The NHS and other public organisations supply us with information relating to things like emergency department attendances, hospital admissions, general practice visits and education records. Organisations do this in order to contribute to research and ultimately improve service provision in the UK. Just like the information gathered via questionnaires this data is fully anonymised and encrypted.

Please consent if you are happy for your questionnaire data to be linked with routine data. We will need a few details from you (name, address, gender and date of birth) in order to make the link with this routinely collected information. You will never be contacted at your address. Once this link has been made your name address, and postcode will be removed so that you cannot be identified, even by the research team.
